Community Health Care
The Community Health Care Initiative is a GRRO-led effort to strengthen health outcomes in rural communities through innovative, community-rooted care. By addressing the social and environmental factors that influence health, this initiative expands access 15 rural counties to activate preventive care, chronic disease management, and wellness services for underserved populations. Through collaboration with local providers, schools, and support agencies, the initiative builds resilient systems of care that empower individuals and families to lead healthier, more stable lives—transforming health equity from aspiration to reality in rural regions.
