The Center of Excellence for Rural Health is committed to improving the well-being of rural communities by connecting community members to resources that address key health challenges such as substance use, mental and behavioral health, and physical health disparities. Through innovative programs and strategic partnerships, the Institute works to support local health providers in delivering accessible, effective care. Our mission is to empower rural areas with the resources and solutions needed to reduce chronic conditions, promote mental wellness, and mitigate the impacts of substance use, creating healthier, more resilient communities.

Healing Hearts @ Home
This program bridges the gap between medical care and the social challenges that patients face in rural settings. By embedding a Healthcare Navigator within the hospital’s interdisciplinary team (IDT), we ensure that patients receive essential social support, including assistance with transportation, food security, and post-acute care guidance.

Food is Medicine is a transformative GRRO initiative that integrates access to fresh, healthy foods into the continuum of care for rural communities.
