Sarah Frei
Health Related Social Needs (HRSN) Director
Vaya Partners

Sarah Frei is a healthcare and population health leader with a passion for building stronger connections between healthcare and the communities people call home. Her work focuses on addressing social drivers of health, including food insecurity, housing instability, transportation, and access to care, with a particular focus on the unique needs of rural communities.

Through her work at Vaya Partners (previously Vaya Health), Sarah has led Medicaid transformation and community-based initiatives that bring healthcare organizations, public health, and community organizations together to improve access to essential resources and ultimately improve health outcomes. She has been deeply involved in North Carolina’s Healthy Opportunities and Rural Health Transformation efforts, helping translate policy and funding into practical, community-level solutions.

Sarah brings a cross-sector perspective and a strong belief that food, housing, transportation, and other basic needs are not separate from healthcare; they are fundamental to health.
